DRIVER POLICY SIGN OFF

Universal Staffing Policies below are for all drivers placed on assignment. By signing here you show understanding that you must abide and adhere to all policies while on assignment. If it is found that you do not follow all policies set forth there will be action taken of progressive discipline from removal from the current assignment to immediate termination depending on the severity of the misconduct.

DRUG AND ALCOHOL POLICY

Universal Staffing prohibits any use, distribution, dispensation, or possession of a controlled substance or alcohol while on assignment. This is a zero tolerance policy.

EMERGENCY AND ACCIDENT PROCEDURES POLICY

All drivers placed out on a driving assignment must report any and all emergencies and accidents immediately to both their Universal Staffing representative and to their assigned worksite supervisor. The Driver should also be aware of the Customer’s Emergency and Accident Procedures and follow such procedures as accordingly.

PRE-TRIP INSPECTION POLICY

All drivers placed out on a driving assignment must complete a Daily Inspection Sheet (Pre –Trip) prior to operating the vehicle they have been assigned to. If the Driver finds that the vehicle is unsafe or has some defects, the Driver should immediately inform his direct supervisor/dispatcher of the defects or concerns regarding the assigned vehicle before operation of the vehicle. The assigned driver has the right to refuse work and should not operate a vehicle that they have deemed unsafe. If the Driver feels that the direct supervisor/dispatcher has not dealt with his/her concerns, they should immediately call their Universal Staffing representative and explain their concerns.

HOURS OF WORK/LOG BOOK POLICY

All drivers placed out on a driving assignment must have a full understanding of the Ministry of Transportation’s rules and regulations with regards to Hours of Work and Log Book Legislation. All Drivers working for Universal Staffing on assignment shall abide by all rules and regulations set forth by the Ministry of Transportation and make available to Universal Staffing any and all records pertaining to their Hours of Work/Log Book.

VEHICLE USAGE POLICY

All drivers placed out on a driving assignment utilizing a customer provided vehicle are not to use the vehicle for any personal use or gain. Customer Vehicles are not to be outside of your scheduled shift for personal reasons. All reports of misuse of company vehicles or equipment will be fully investigated. This is a zero tolerance policy.

REPORTING INCIDENTS USAGE POLICY

All drivers placed out on a driving assignment to a Customer must report all issues immediately relating to the operation of the vehicle to Universal Staffing and the Customer appointed supervisor. This includes any violations, tickets, preventable and non-preventable incidents. It is the responsibility of the driver under the Motor Vehicle Act.

CELL PHONES AND OTHER TRAFFIC TICKETS/FINES/INCIDENTS

All drivers must follow all road traffic laws pertaining to Cell phone usage, carrying their Drivers License at all times and all other provincial traffic laws. All drivers are responsible to report any address changes within thirty days to the Ministry of Transportation. Failure to comply with this policy will result in fines being the responsibly of the driver to pay.

VEHICLE ISSUED EQUIPMENT/TOOLS/CREDIT CARDS POLICY

All drivers using a radio for communication must act accordingly. Radio usage and communications are to be professional at all times. There is to be no profanity, racist, or otherwise offensive language over the frequency. This is a vital tool of operation and must be treated as such. Cellular phones issued are to be used for company related business only. Cellular phones are strictly monitored. Incoming and outgoing personal calls are not permitted. Cellphones are not to be used while driving. Fuel cards issued are to be used for operational purposes only. You must ensure when fueling that you are using the proper type and grade of fuel. If you are unclear as to what type of fuel should be used, contact your direct supervisor/dispatcher. Company issued equipment and tools should never be used for personal use. If deemed there is misuse there will be actions taken to recollect any monies deemed non work related.

SMOKE-FREE ONTARIO ACT / WORKPLACE VEHICLES

All drivers are to abide by the Ontario Law that prohibits smoking in a workplace and workplace vehicles. A Workplace vehicle is a vehicle used by an employer or employee to conduct any type of work in an enclosed workplace under the Smoke-Free Ontario Act. (SFOA) Similar to no smoking inside an office building, smoking is prohibited in the workplace vehicles, 24 hours a day/7 days a week, before or after work hours and with or without passengers. Ashtrays, or equivalent, are NOT to be utilized for smoking purposes. There will be a No Smoking Sign clearly placed in all workplace vehicles visible to all occupants. Be aware there are fines - FINES: A penalty fine will be applied if any parties do not comply with this law.

Employee: Fine ranges from $250.00-$5,000.00. Each Company will have a designated fine amount. Please check with your Universal Staffing representative. Non-Driver positions are prohibited from smoking in or around all workplace vehicles as well as per company policy. This includes all Loader/Thrower positions.
